Work Permits
Work Permit Application Instructions:
- Download the Work Permit Application.
- Work Permit Application must be signed by Parent and Employer.
- Once completed with signatures, please drop off the application to Victoria Maldonado in the COHS Student Services Office.
- You will receive a phone call and/or email when your Work Permit is ready to be picked up in the Student Services Office.
